Appropriate airway management is an integral part of anaesthesiologist’s role. Huge goitres can lead to distorted and difficult airway and difficulty in endotracheal intubation. In this report, we present a case of a 65–year–old  male with a huge multinodular thyroid swelling, gradually increasing in size for last 10 years. He had a history of change in voice. History of dysphagia and dyspnea on exertion was also present. He was diagnosed as a case of multinodular colloid goitre (MNG) , advised surgery, and was admitted for the same.

A close working relationship between anesthesiologists and surgeons was the key to the safe use of anesthesia and uneventful recovery of this patient. where trachea was successfully intubated by awake fibreoptic intubation technique.
