Background & Aims: The development of pulmonary hypertension in association with elevated pressure in portal circulation is known as portopulmonary hypertension(POPH). POPH is the fourth most common form of pulmonary artery hypertension(PAH). Hence the aim of this study is to evaluate prevalence of POPH, grading severity of POPH and its association with body mass index(BMI), diabetes, systemic hypertension, smoking and right ventricular dysfunction.

Methods: study was conducted on 147 patients with features of portal hypertension. Trans–thoracic echocardiography was done for calculation of PASP (pulmonary artery systolic pressure) and TAPSE(tricuspid annulus plane systolic excursion), TDI( tissue Doppler imaging) S’  for right ventricular dysfunction(RVD) . Patients with PASP more than 35mmhg underwent right heart catheterization for measuring mean pulmonary artery pressure(mPAP) and mPAP of more than 25mmhg confirmed POPH.

 Results: Prevalence of POPH   was  8.1%%, in male and female it was 8.2% and  8.0% respectively. Sex was insignificant in POPH with p–value of 0.974.  Mean age of POPH patients were 53.33+ 8.3 years, age was insignificant with p–value 0.86. Body mass index(BMI), hypertension, diabetes and RVD was found significant in POPH patients with p–value of 0.01, 0.008, 0.05 and 0.0001 respectively. Smoking was insignificant (P–0.971).

Conclusion: This study points towards increasing trends in the prevalence of POPH and its association with obesity, hypertension, and diabetes.
