Nodular medulloblastoma is an embryonal neural tumour arising in the cerebellum and characterized by nodular, reticulin–free zones and intervening densely packed, poorly differentiated cells that produce an intercellular network of reticulin–positive collagen fibres. A 23 yrs. old male patient had a midline cerebellar SOL adjacent to the tentorium. The patient was operated and the lesion was excised. Histopathological examination showed multiple pale areas (nodules) having large cells with euchromatic nucleus in a fibrillaryneuropil like background and the nodules were surrounded by densely packed densely packed, undifferentiated, highly proliferative cells with hyperchromatic and moderately pleomorphic nuclei. The nodules in desmoplastic/nodular medulloblastoma show variable expression of neuronal markers, including synaptophysin and NeuN. Desmoplastic/nodular medulloblastoma displays pathological activation of the SHH pathway, which is often caused by mutations in genes encoding members of the pathway, including PTCH1, SMO, and SUFU.
