 The CT scan plays an important adjunct to define pathology, severity of head injury and is essential in the ongoing management.

MATHODS: A total of 104 cases of traumatic brain injury (TBI) patients   who met with inclusion and exclusion criteria were included in the study. Patients were managed, initial CT scan (<12 hours of injury) and serial CT  scan (<48 hours of injury) were done. Data was recorded, compared with neurological status and glasgow outcome scale(GOS).

RESULTS: Persistence of symptoms of TBI had significant association with change in Marshall and modified Marshall grading (p<0.0001, p=0.010). GOS had association with change in CT–Marshall grading but not with change in Modified Marshall. Change in neurological status had significant association with change in Marshall grading (p<0.0001).

CONCLUSION: The change in Marshall or Modified Marshall grading together with change in neurological status can improve the accuracy of detection of operative surgical lesions.
