Percutaneous  Transhepatic  Biliary Drainage is usually done with ultrasound and fluoroscopy combined guidance for relief of obstructive jaundice  In our study only ultrasound guided and single step percutaneous  transhepatic biliary drainage was performed for relief of obstructive jaundice  which is very efficient and cost effective. It is  very cost effective  and feasible at low infrastructure  set up (can be feasible at secondary level of health centres )with good outcome and very effective  in decreasing the  morbidity and mortality of non operable cases of obstructive jaundice
