Peripartum cardiomyopathy (PPCM) is a rare cardiomyopathy defined as systolic cardiac heart failure in the last month of pregnancy or within five months of delivery without any identifiable cause.The precise incidence in India is not known , except for which quotes as 1 per 1374 live births from a tertiary care hospital from South India .(2 ) Though PPCM resembles dilated cardiomyopathy clinically left ventricle may not be always dilated. (3,4,5).

Here we report a caseof 34 wks ANC with severe preclampsia with predelivery LVEF 25–30 % with systolic heart failure who had a good fetomaternal outcome .Patient presented with acute breathlessness with orthopnoea .Patient recieved intensive care management with ventilatory support and delivered by emergency LSCS. After postoperative hospital stay of 25 days patient went home with baby .
