BACKGROUND: Image guided neurosurgery provides a patient specific three dimensional anatomy for preoperative planning and intra–operative navigation thus helping surgeon to perform complicated procedures with improved accuracy and safety.

METHODS: This is a prospective interventional study of patients with intracranial lesions over a period of 18 months .A total of 60 Patients  with non traumatic intracranial lesions were included in the study which were equally divided amongst IGS and conventional surgery groups. The data was analysed using SPSS software version 17. P value of 0.05 was considered as statistically significant.

RESULTS: Length  of  incision,  accuracy of placing craniotomy,  post  operative  pain and radicality of excision  is significantly  improved  by the use of IGS.

CONCLUSION: Neuronavigation should be routinely used in planning incision and craniotomy in supra tentorial  lesions as  its usage  adds up to significant  benefits for the patients.
