Introduction: Chronic ankle and foot pain is common and non–specific presentation of the broad spectrum of osseous and soft tissue pathology of ankle and foot. In appropriate clinical setting MRI help to diagnosis of these conditions and manages conservatively. Aims: Assessment of the normal anatomy and anatomical variation MRI features provided vital clues about diagnosis of the painful ankle and foot. 2. To learn characteristic MRI imaging features of painful conditions of the ankle and foot and differentiate these complication from each other. 3. To enumerate various cause of the painful ankle and foot Materials and methods: This is Descriptive type of study of the total 50 patient presenting with pain in the ankle and foot in the department of the radiodiagnosis for MRI, MBG Hospital and RNT Medical Collage from November 2017 to October 2018. Results: In our study commonest cause of chronic pain in ankle and foot was ligament pathology found in 12(24%) patients out of 50, followed by tendon injuries found in 10 (20%) patients. Other cause includes arthritis, bursitis osteomyelitis and ganglion cysts. The commonest age group affected in our study was 21 to 30 years. Conclusion: MRI is modality of the choice for evaluation and characterization of the various pathology of the ankle. Most of the conditions are managed conservatively.
