Objectives: Displaced fracture of zygomatic arch warrants open reduction and internal fixation. However, approach to zygomatic arch can cause nerve damage, temporal hollowing etc. This article studies the safety and efficacy of hemicoronal approach using supratemporalis plane of dissection in treating zygomatic arch fractures. Materials and methods: 20 patients seeking treatment for zygomatic arch fracture were selected for this study. All underwent surgery for open reduction & internal fixation using hemicoronal approach and supratemporalis dissection. Post–operative facial nerve function, neurosensory deficit, temporal hollowing, scar formation, regional alopecia etc. were evaluated. Results: A total of 20 patients were included in our study. Postoperatively, 3 (15%) patients manifested transient facial nerve. However, at the 3 months post–operative follow–up, all 20(100%) of them showed normal facial nerve function. One (5%) patient developed mild scalp haematoma. No major scar formation occurred on the incision line. The incision line over the scalp was well hidden by hair–growth at the 3 months post–operative follow–up, the endaural extensions were inconspicuous. No subject showed regional alopecia or temporal hollowing. Conclusion: Hemicoronal incision with supratemporalis approach is a relatively safe procedure for open reduction and internal fixation of zygomatic arch fracture.
