Among all the sites of metastasis of visceral tumor, cutaneous involvement is a rare manifestation with 10% incidence. Cutaneous metastasis often present very late in the disease course with average time interval has been reported to be 33 month. Skin metastases usually indicate a worse prognosis. Unusual metastatic location like skin, stomach is clinically important. Here is a case report of 26–years–old male of signet cell carcinoma of stomach presented with features of gastric outlet obstruction having multiple firms to hard nodules all over abdominal skin and scalp. He was initially managed conservatively and routine blood investigation done. His ultrasound, CECT (whole abdomen) suggested of gastric carcinoma. Upper GI endoscopy biopsy confirmed it as a case of Signet Cell Carcinoma of stomach. Excision biopsy of one of the skin nodules was done and was found to be a cutaneous metastasis. The patient did not turned up for surgery and was referred to higher centre on his request.
