Introduction: Leprosy is a leading cause of permanent disability. Claw hand deformity caused by involvement of Median and ulnar nerve, compromises the vocational activities. Surgeries aim in stabilization of the metacarpo–phalangeal joint in flexion, to prevent it from hyperextension. This study compared the outcomes & operative time after Palmaris Longus 4 tail & FDS tendon transfer in ulnar claw hand. Material and methods: A total of 40 Patients meeting inclusion criteria were selected into 2 groups I.e. Patients undergoing Palmaris Longus 4 tail tendon transfer (PL4T) and FDS tendon Transfer. Results: Functional improvement in PL4T is more than in FDS. Though operative time in FDS group is shorter than PL4T group but statistical analysis does not show any significance. Conclusion: Measurement aspect of both groups in this study failed to reveal any difference of statistical significance.
