Prediction of a difficult Laparoscopic cholecystectomy would allow the surgeon to discuss the likelihood of conversion to Open cholecystectomy with the patient and prepare them psychologically. The aim of the study is to preoperatively predict the conversion of Laparoscopic cholecystectomy using clinical, hematological, and radiological factors. A total of 50 patients with cholelithiasis admitted at ASRAM General hospital, Eluru over one year, and were planned for Laparoscopic cholecystectomy were included in this study. Patient factors, Haematological factors, and Ultrasonographic factors were analyzed. Increasing age, higher BMI, previous surgeries, increased total bilirubin, increased gallbladder wall thickness, presence of pericholecystic collection, liver cirrhosis were associated with increased risk of conversion to Open cholecystectomy. The results demonstrate that these preoperative factors can predict difficulty to be encountered during Laparoscopic cholecystectomy and help in making a decision for conversion to Open procedure thereby preventing unnecessary complications.
