Pulp canal calcification is challenging task because determining the correct location of root canal is difficult. Using a “guided concept”, a minimally invasive access is achieved without risk of iatrogenic damage. This case report describes a 28 years old male patient with the chief complain of pain and discolouration in relation to 11 tooth. Periapical radiograph and CBCT reveal severe pulp canal calcification and signs of apical periodontitis. Tooth didn’t respond to thermal and electrical sensitivity tests. So, decision was taken to perform “Microguided Endodontics” using 3D printed guide. To facilate this technique, A CBCT and intraoral surface scan were aligned using special software which allow virtual planning of optimal access cavity up to the apical third of root. Root canal treatment initiated with the aid of 3D printed guide using an EndoGuide bur –5. After negotiation of root canal, conventional root canal treatment was performed under microscope.
