The mandibular ramus is regarded as a relatively safe zone for the surgical procedure. The only important anatomical structure is mandibular canal. The ramus has a anatomical variant is temporal crest canal. (TCC) TCC was inferred to have conveyed all or part of the buccal nerve. The clinical significance of this variant is that pain fibres from the molar roots joining the nerve as it runs in this canal would exit the bone behind the temporal crest, thus escaping anesthetization in routine injections for inferior alveolar and buccal nerve block. Also lack of its awareness can complicate surgical procedure. Recently high resolution cone beam computed tomography (C.B.C.T.) has been widely used for achieving an imaging diagnosis of abnormalities in the oral and maxillofacial region. So this rare clinically significant anatomical variation can be carefully investigated using reconstructed C.B.C.T. images
