Twin reversal arterial perfusion (TRAP) is a rare sequence of monochorionic twin pregnancies affecting 1% of monochorionic pregnancies. It arises when the cardiac system of one twin does the work of supplying blood to both the twins through an artery to artery anastomosis. Supplying twin is known as ‘pump twin’ and receiving twin which lacks a heart is known as ‘acardiac twin’. We present a case in a 23 year female, gravida two, para one, living one, preterm pregnancy, ultrasound showed twin pregnancy with TRAP sequence. The first fetus was delivered vaginally and the second fetus by cesarean section. Both the twins had congenital anomaly and did not survive.
