Osteoporotic Intertrochantric fractures are a serious health concern and a notable burden on healthcare systems. Faster time to union results in decreased complications and less morbidity. Teriparatide has been shown to expedite fracture union but there is sparse literature on this. All the patients who had suffered intertrochantric fractures and were treated with PFN between February 2019 to January 2020 were included in this prospective study to evaluate the effects of teriparatide on time to union and functional recovery. Patients were divided in two groups .Group 1 did not receive teriparatide postoperatively while the Group 2 received teriparatide. Patients were then followed up and evaluated on the basis of radiological union, functional outcome, pain score and the need for revision surgery. A total of 98 patients met our inclusion criterion. 8 patients were lost to follow up. Of these 42 patients received teriparatide and 48 patients did not receive teriparatide. A total of 90 patients were available for analysis. Time to union in Group 1 was longer (14.6 weeks) compared to the Group 2 (11.1 weeks).Patients in Group 2 also showed better results in terms of hip pain. Mean hip pain scores in Group 1 and 2 at 3 months were ( 2.6 and 1.4 ),and at 6 months were ( 2.4 and 1.2) respectively. Functional recovery in terms of ability to move inside and outside the house was also significantly better in Group 2 patients. These patients also showed less radiologic complications in terms of varus collapse and less implant cut out rate. We concluded that teriparatide was an effective adjunct therapy in osteoporotic fractures treated with PFN
