Bronchoalveolar lavage(BAL) fluid analysis helps in early detection, rapid diagnosis of lung cancer . The accuracy of diagnosis on the basis of BAL fluid cytologyof lung neoplastic lesions are being confirmed by the histological study of the biopsied specimen. AIM– This study aims to detect diagnostic accuracy of BAL Fluid cytology in detection of lung cancers as compared to their respective biopsied specimen. MATERIAL AND METHODS– It is a retrospective study in a tertiary care centre involving analysis of BAL fluid cytology and comparison of results with biopsy from JANUARY 2019 to JANUARY 2020 RESULTS– Out of 54 cases analysed, BAL fluid showed 37 positive cases for malignancy and 48 cases were positive in biopsied specimen. Adenocarcinomas were most common cancer noted in our study. BAL fluid was falsely negative in 11 cases proved by lung biopsy with a sensitivity of 77% and specificity of 89%. So the diagnostic accuracy is 75.9%. So BAL fluid examination is of earliest diagnostic value as compared to gold standard histopathology. CONCLUSION–BAL fluid analysis provides a rapid, reliable process to detect and subtype malignancies of lung in clinicoradiologically suspected cases
