This is case of non–malignant choriocarcinoma in the patient that had been operated two years back for silent rupture of the rudimentary horn of bicornuate uterus due to perforating mole. In the previous surgery, the horn was excised and uterus kept to preserve the fertility. However, patient was lost to follow–up and further management could not be provided. History of hydatidiform mole increases the risk of choriocarcinoma in subsequent pregnancy. Currently patient presented with 4 months amenorrhea. Ultrasonography of abdomen and pelvis revealed that uterus was enlarged and a heterogenous mass was seen inside the uterus with no evidence of amniotic sac or fetal parts. Also posterior enhancement was seen. A probable diagnosis of choriocarcinoma or invasive mole was made. Intra operatively uterus was found to be grossly destroyed with tumour growing in all directions. This warranted obstetric hysterectomy. Histology of the tumor suggested non–malignant choriocarcinoma. Post–operatively, patient was started on methotrexate and folinic acid, however she was lost to follow–up after single cycle.
