From the first day when the patient gets his/ her Orthodontic treatment started, the most common and repeated question which the patient asks their orthodontists is “When will my braces be removed”. This means the duration of the orthodontic treat–ment is crucial for the patient’s happiness. In order to satisfy the ever demanding ortho–dontic patient, many researchers have proposed various techniques to reduce the over–all treatment duration. To name a few, Wilckodontics[1], Micro–Osteoperforations[2], Low level–laser therapy[3] & PAOO[4]. In this case we have used the Surgery first concept which was introduced by by Brachvogel[5] in 1991, in order to reduce some of the dis–advantages and negative outlook of pre–surgical orthodontics. This case showcases the successful implementation of the surgery first concept in treating an individual with prognathic maxilla accompanied with a gummy smile.
