Epithelial blistering as seen in chronic ulceration in oral cavity, when not diagnosed and treated well in time, could result in severe extra–oral manifestation of the condition. The current case report deals with one such recalcitrant case, confirmed as Pemphigus Vulgaris on histo–pathological evaluation, is being discussed. Along with initial oral lesions, extra oral lesions on scalp, back and genital mucosa were also noted. Topical triamcinolone and systemic prednisolone were instituted along with anti–fungal mouth rinse. Re–establishment of normal mucosa was achieved post treatment. Early identification of such lesion by proper diagnosis and appropriate treatment could be helpful in preventing potentioal dermatological conditions
