To Determine The Predictors Of Nonalcoholic Fatty Liver Disease And Nonalcoholic Steatohepatitits In Patients With Type 2 Diabetes Mellitus

Abstract :

Nonalcoholic fatty liver disease (NAFLD) refers to the accumulation of fat mainly triglycerides in hepatocytes that results from insulin resistance. It is the most common chronic liver disease in the western world. The clinicopathologic spectrum of NAFLD ranges from bland hepatic steatosis which is clinically associated with similar long term prognosis as compared to general population to nonalcoholic steatohepatitis (NASH) which when associated with increase liver fiosis, may progress to cirrhosis and liver failure as such distinguishing between NASH with and without fiosis has important implications for management and patient counseling. Aims and objectives The aim of the study is to determine the predictors of nonalcoholic fatty liver disease and nonalcoholic steatohepatitis in patients with type 2 diabetes mellitus. Materials and methods The study consisted of evaluating fiosure as diagnostic and prognostic test for NAFLD disease taking liver biopsy as gold standard. The study was conducted on 50 inpatients of department of medicine, patna medical college and hospital patna from october 2016 to September 2018 who fulfilled inclusion and exclusion criteria. Summary and conclusion The study to evaluate fio sure test as diagnostic modality for diagnosis of NAFLD taking liver biopsy as gold standard has given following conclusions : In patients with provisional diagnosis of nonalcoholic fatty liver disease based on clinical history and radiological investigation fiosure test can be used as diagnostic test and also for staging of the disease since it has very high sensitivity and positive predictive value and higher fiotest scores correlate well with higher stages of fiosis on liver biopsy. Definite diagnosis of the condition can only be done on liver biopsy.Fiosure test has both diagnostic value as well as prognostic value. This is because of the fact at higher score of fiotest the sensitivity and positive predictive value of the test is close tyo ideal diagnostic test and there is direct relationship between fioscore and degree of fiosis on liver liver biopsy.
