Epidemiology and Management of paediatric epistaxis in Ekiti

Abstract :
Background/ Objectives:Epistaxis is a common presentation in childrenandmayconstituteamajortroubletotheparentsandthetreatingdoctors.
This study aimed at determined the clinicoepidemiological patterns and management of epistaxis.
Materials and Methods:Thisisaprospectivehospitalbasedstudyofchildrenwithnasalbleeding in our center.Thestudywascarriedoutfrom June2012toJuly2017.
Consented patients were enrolled into the study. Thedatawascollectedusingapre–testedinterviewerassistedquestionnaire.
ThedatacollectedwerecollatedandanalyzedusingSPSSversion16.0.
Results:In this study prevalenceofepistaxisinchildrenwas7.1%.Themodalagegroupwas6–10yearswithepistaxisprevalenceof34.3%.
Therewere67.6%malesandmaletofemaleratioof2:1.
Inthisstudyanteriorepistaxiswas71.6%andcommonerthanposteriorepistaxisin17.6%.Singleepisodeofepistaxiswas84.3%overrecurrentepistaxisin15.7%.Acuteepistaxisin89.2%wascommonerthanchronicepistaxisin10.8%.Unilateralepistaxiswas58.9%andrightnasalbleedingaccountedfor31.4%.
Localcausesofepistaxisaccountedfor94.1%.Maincausesofepistaxiswastraumain45.1%followedbyinfectiverhinosinusitisin27.5%.Lesscommoncausesofnosebleedingweresepticeamia,sicklecelldiseasesandidiopathicin1.0%,2.9%and5.9%respectively.
Epistaxisleadstoabsenteeismin80.4%andirritablein74.5%.
Therewereassociatedvomitingin30.4%andmildanaemiain20.6%.Majority85.3%weremanagedbyobservationalone.Theoutcomeoftreatmentwasgoodrecoveryin90.2%whilepatientssatisfactionwithtreatmentwasnotedin84.3%.
Conclusion: Epistaxis is commoninchildren with effect on their quality of life.Commonest site of nose bleeds in children is local origin mainly from traumatic and inflammatory aetiology.
