A COMPARATIVE STUDY OF CLINICAL EXAMINATION SCORES, SEMMES- WEINSTEIN MONOFILAMENT EXAMINATION, VIBRATION PERCEPTION THRESHOLD AND NERVE CONDUCTION STUDIES IN THE DIAGNOSIS OF DIABETIC POLYNEUROPATHY.

Abstract :

Diabetic neuropathy(DN) is one of the most common long–term complications of diabetes . This study aims to asses and compare Diabetic Neuropathy Examination Score(DNE), Semmes –Weinstein monofilament examination(SWME), Viation perception threshold(VPT) and Nerve conduction studies(NCS) in the diagnosis of DN. METHODS :100 diabetic patients were subjected to Diabetic Neuropathy Symptom Score (DNS) and those subjects with positive DNS score were subjected to DNE ,SWME,VPT and NCS. NCS was taken as gold standard and sensitivity and specificity for DNE, SWME, VPT were calculated. RESULTS: Out of 100 subjects 78 had DN confirmed by NCS. The sensitivity of DNE, SWME and VPT were 85.9%, 87.2% and 91% respectively. The specificity of DNE, SWME and VPT were 68.2%, 63.6% and 72.7% respectively. CONCLUSION: Simple neurological bedside clinical scores and tests are useful screening tools in diagnosing DN in view of the ease of applicability and cost effectiveness.
