A prospective study on the effect of combining intraoperative infiltration of 0.25% bupivacaine hydrochloride with intramuscular ketorolac for immediate postoperative analgesia in orthognathic surgery

Abstract :

Aim: Compå the efficacy of administering a combination of local infiltration with bupivacaine HCl 0.25% and intramuscular. Materials and methods: This study was conducted at the Department of Oral and Maxillofacial Surgery, Saveetha Dental College and Hospital, Saveetha University, Chennai. Seventy patients who underwent bi–jaw orthognathic surgery under general anaesthesia were included in this study. The surgical procedures were performed by surgeons with similar levels of experience, identical operating conditions and using standardized techniques. Any patient who had to undergo additional procedures were excluded from the study. Patients allotted to the Study Group received 0.25% Bupivacaine HCl with 1: 200000 concentration of adrenaline solution as infiltration at the surgical site and a dose of IM ketorolac 30 mg. Patients who had been allotted to the control group were given IM ketorolac 30 mg alone. Results: Comparison of study and control group with respect to the number of minutes passed before the first rescue analgesic administration, the total number of analgesics administered at the end of 72 hours and with the overall VAS scale was done with an independent t–test. The p–values were <0.0001, <0.0030, and <0.003 which showed highly significant statistical values. Conclusion: Our study showed that intraoperative wound infiltration with 0.25% bupivacaine was effective in reducing the pain and analgesic consumption by the patient in the postoperative period.
