A STUDY OF RISK FACTORS, CLINICAL PROFILE AND OUTCOME OF NEW AND ESTABLISHED TYPE 1 DIABETES MELLITUS PRESENTED WITH DIABETIC KETOACIDOSIS IN CHILDREN LESS THAN 14 YEARS IN A TERTIARY CARE HOSPITAL

Abstract :

Diabetic ketoacidosis is an acute metabolic complication of DM with heterogeneous clinical presentation. It classically occurs in young patient with type 1 diabetes mellitus (T1DM). Its incidence is increasing .This study was undertaken in a tertiary care hospital (L.G. hospital)Ahmedabad on 42 cases from 1ST June 2018 to 1ST July 2019.Diagnosis was done according to ISPAD criteria for DKA and treated accordingly.[1][2] Aim of the study was to identify various risk factors for triggering the events of DKA and to study various presenting symptoms, severity and time taken to recover from DKA. Out of total 42 patients of DKA, 2 patients were from <2 years, 8 were between 2–5 years,22 were between 5–10 years of age group and 10 were from 10–14 years of age group. All patients below 5 years of age presented with DKA for the first time. In this study group, more female patients were presented with DKA. Most common precipitating factor was infection in 43%. Among the established T1DM, noncompliance was found to be precipitating cause in 40% patients. Most common presenting symptom among new cases were difficulty in eathing where as in patients having diagnosed T1DM, it was abdominal pain and vomiting. Most of patients were having moderate acidosis and recovered within 12 hours of admission in our study.
