A STUDY TO DETERMINE THE EFFECT OF SPLENECTOMY ON THROMBOTIC EVENTS IN HbE-BETA THALASSEMIA PATIENTS

Abstract :

In the present study we tried to find out the correlation of splenectomy with thrombotic events in the form of portal vein thrombosis & pulmonary hypertension in Hb–E Beta thalassemia patients. Our study was hospital based cross sectional comparative study. Study population included 15 splenectomized E–Beta thalassemia patients (defined as case) & equal number of age & sex matched non–splenectomized E–Beta thalassemia patients (defined as control) who agreed to be part of the study. Keeping in agreement with the inclusion criteria all patients were more than 12yrs old. Doppler echocardiography revealed the presence of pulmonary hypertension (defined as PASP≥35 mmHg) in a total number of 9 patients out of which 7 were splenectomized & 2 were not. However Right Heart Catheterisation confirmed the presence of pulmonary arterial hypertension in 5 patients out of those 9. This time it included 4 splenectomized & 1 non–splenectomized patients. All these patients had normal pulmonary capillary wedge pressure which excluded left heart pathology as an etiology. Although the above observation clearly states that both the events were more prevalent in splenectomized group however standard statistical analysis did not reveal any significant difference between two groups (splenectomized & non–splenectomized) in respect to both the events. Both the events were found to be more prevalent in patients who were non–transfusion dependent & either chelation–na�ve or on irregular chelation.
