Comparison of corneal higher order aberrations (HOA)in eyes with regular astigmatism and eyes with mild to moderate keratoconus

Abstract :

PURPOSE : Purpose of this study was to compare corneal higher order aberrations (HOA) of eyes with regular high astigmatism to that with mild to moderate keratoconus. METHODS : Fifty eyes were included in this Prospective observational comparative study. Out of 50 eyes, 25 eyes were having regular high astigmatism (group 1) and 25 eyes showed changes of mild to moderate keratoconus (based on Amsler–Krumeich classification) (group 2). Topographic evalution of all the eyes was done on Zeiss Atlas 9000 Topograher. Mean age of the patients, keratometry values and high order aberrations like coma, trefoil, spherical and oblique measurements, CIM, TKM, shape factor were measured and compaired using paired t test. RESULTS : There was no statistical difference in terms of age and gender distributions between two groups (p>0.005). However there was statistical difference between the groups in terms of HOA like horizontal coma , vertical coma , oblique trefoil, horizontal trefoil, spherical , oblique quarterfoil, oblique secondary astigmatism ,CIM, TKM, shape factor (p<0.001) except oblique astigmatism (>0.005%). CONCLUSION : According to our study corneal higher order aberrations other than oblique astigmatism were clinically and topographically higher in keratoconic eyes than normal high astigmatic eyes. This findings may be due to asymmetrical and morphological changes in the stage of keratoconus leading to deterioration in the indicators of corneal irregularity. Anterior higher order aberrations are a useful tool to guide the physician in the diagnosis and classification of keratoconus .
