Comparison of oral clonidine vs Interscalene block on perioperative haemodynamic response and postoperative analgesic requirement in patients undergoing Shoulder Arthroscopic Surgery under General Anaesthesia

Abstract :

 Shoulder arthroscopic surgery is a minimally invasive surgery that is done to examine or repair the tissues inside or around shoulder joint. Bleeding is a frequent complication that limits the surgeon’s field of view and affects the operative technique so hypotensive anaesthesia is commonly used  to decrease bleeding and improve visualization. Another problem of shoulder surgery is severe postoperative pain.  If postoperative pain and intraoperative bleeding can be taken care of by a single method then patient compliance will be better. The purpose of this study was to investigate the effect of oral clonidine as premedication vs Interscalene block given preoperatively on perioperative haemodynamic response and postoperative analgesic requirement. Fifty patients scheduled for shoulder arthroscopic surgery under general anaesthesia were randomly allocated to receive premedication with either Interscalene block (Group B, n = 25) or oral clonidine 2 – 3 μg/kg (Group C, n = 25) 30 minutes prior to induction. The two groups were compared with respect to haemodynamic parameters, Arthroscopic Visualization Score, Propofol requirement, pain and sedation scores, time to request of analgesic and cumulative analgesic requirements. Oral clonidine was found to be significantly better in terms of maintaining stable haemodynamics, good Arthroscopic Visualization Score and having a Propofol spå effect compared to the Interscalene  group, though time interval to the first request of analgesia postoperatively were nearly same in the two groups i.e. about 4 and half  hrs.
