CYPROTERONE ACETATE INDUCED PAPILLOEDEMA - A RARE CASE REPORT

Abstract :

Papilledema is the optic disc swelling produced by increased intracranial pressure. It is usually bilateral and rarely unilateral. It presented with symptoms such as episodes of transient often bilateral visual loss lasting for seconds mostly precipitated from a lying or sitting position (altering with intracranial pressure); horizontal diplopia,headache; double vision; nausea; projectile vomiting and rarely a decrease in visual acuity( if associated with macular disturbance). Visual field defects and severe loss of central visual acuity occurs mostly with chronic papilledema. It present with signs such as elevation of optic disc ,blurring of optic disc margin, filling in of the physiological cup, edema of the nerve fie layer often obscuring the blood vessels, peripapillary retinal hemorrhages , dilated tortuous retinal veins retinal or choroidal folds (Paton’s line) and macular fan. It is classified into early, established, chronic and atrophic stage based on its clinical signs. It is caused by conditions like primary metastatic intracranial tumours , hydrocephalus, idiopathic intracranial hypertension, subarachanoid hemorrhages, subdural & epidural hematomas, A–V malformations, ain abscess, meningitis and encephalitis. In this case patient was diagnosed with both eye established papilledema , rarely caused by an antiandrogen medication namely cyproterone acetate used in the treatment for infertility due to polycystic ovarian syndrome
