Dry Eye Disease among Rheumatoid Arthritis Patients

Abstract :

Purpose: The aim of the study was to investigate the prevalence of dry eye disease (DED) among rheumatoid arthritis (RA) patients and to evaluate the association of rheumatoid arthritis disease severity and its duration with the DED. Material and Methods: This was a randomized cross–sectional study of 82 randomly selected patients between 21 to 60 years of age diagnosed to have RA disease, age and sex matched 82 normal persons as controls. The severity of RA disease was calculated using a formula of the disease activity score (DAS28). Standard patient evaluation of eye dryness (SPEED) questionnaire, Schirmer’s test and tear film eak–up time (TFBUT), Rose Bengal staining tests were used for diagnosis of DED. Results: Out of total 82 patients with RA, 13(15.85 %) were males and 69(84.15%) were female. The DAS28 score of RA patients was ranged from 2.84 to 8.44. The prevalence of DED found to be in 28 (34.14%) patients with RA and in 11(13.41%) cases of controls according to the study design. The mean RA disease activity score was 4.7±1.2 in patients with DED and it was 3.9±1.1 in patients without DED. There was no statistically significant correlation between the severity of RA disease and presence of DED. But we got greater number of DED in patients with RA disease of longer than 10 years duration. Conclusions: In the Indian population, RA patients have a significantly higher prevalence of DED in comparison to age and sex matched controls. The DED is not associated with severity of rheumatoid arthritis but it increases with the duration of arthritis.
