ETIOLOGY AND CLINICAL PATTERN OF OCULAR MOTOR NERVE PALSIES

Abstract :

Ocular motor nerve palsies are one of the leading causes of ocular morbidity in the neuro–ophthalmology clinic. The recognition and localisation of ocular motor nerve palsies causing paresis or paralysis of extraocular muscles is a challenging entity for the ophthalmologist. Objective: To study the aetiology, clinical profile and recovery of ocular motor nerve palsies in a tertiary care centre in South India. Method : Prospective cohort study was done in 75 patients. Inclusion Criteria : All patients attending neuro ophthalmology clinic in Government medical college Thrissur were taken up for study. Exclusion Criteria: Myopathies & Restriction syndromes were excluded. Result: 75 patients were included in the study. Mean age group was 51.37±14.39 years. M:F 1.3:1.Laterality: Left (62%) > Right(34%); 4% Bilateral. Commonest presenting symptom was double vision (76%). Most common nerve involved was 3rd cranial nerve (40%).44% of cases showed first evidence of recovery at 1 month follow up. Discussion: The third cranial nerve palsy was the most common ocular motor nerve palsy in our study. The most common identifiable aetiology was vascular followed by trauma. First evidence of recovery was noticed more in cases with traumatic aetiology. Conclusion: Third nerve palsy was the commonest ocular motor nerve involvement observed in our study. Signs of recovery were evident in all cases of ocular motor nerve palsies during first month follow up except for those due to tumour aetiology
