Evaluation of results of peroneus longus graft in Primary A.C.L Reconstruction : A Prospective Study

Abstract :

 

Objective: To study the scope of Peroneus Longus Tendon (PLT) as an autologous graft in primary ACL reconstruction surgeries and evaluate its possible effect on foot and ankle function.

Material and Methods: Prospective study on 42 patients undergoing a primary ACL reconstruction with a Peroneus Longus Tendon autograft. Post–operative knee function was evaluated by the International Knee Documentation Committee (IKDC) and Modified Lysholm Knee Scoring Scale. The functional status of ankle in follow up visits was evaluated by the Foot and Ankle Disability Index (FADI) score and American Orthopaedic Foot and Ankle Society Score (AOFAS). Foot imprints were obtained by a baropedometer and Staheli’s plantar arch index was calculated to evaluate any loss of integrity of the arches of the foot.

Results: The average diameter and length of the doubled harvested tendon was 8.5mm and 13.5 cm respectively. The mean IKDC scores at 6 months follow up was 86.98 and at 1 year follow up was 93.4 showing an improvement of 50 points from pre–op values. At 6 months follow up, the Lysholm score grading showed ‘Excellent’ in 39 cases and ‘Good’ in 3 case, becoming ‘Excellent’ in all 42 of the cases after 1 year. The mean 6 months and 1 year follow up AOFAS scores were 97.7 and 100 respectively and it remained unchanged thereafter. The mean FADI scores showed similar results. Foot mapping with the use of a Baropedometer and calculating the Staheli’s arch index showed no evidence of loss of integrity of foot arches.

Conclusion: This study showed the efficiency of peroneus longus can be used as a graft for primary A.C.L reconstructions considering its excellent post–operative knee scores. It also established that removing the PLT has no effect on the functioning of the ankle and the foot.
