MEDIAL OPEN WEDGE HIGH TIBIAL OSTEOTOMY FOR OSTEOARTHRITIS OF KNEE : LONG TERM RESULTS IN 30 PATIENTS OF SMIMER, SURAT, GUJARAT

Abstract :

Introduction :To retrospectively study the long term results of high tibial valgus osteotomy in the management of primary medial compartment osteoarthritis, with special reference to patient satisfaction and functional assessment, we conducted this study in radiologically established 30 knees of patients with primary osteoarthritis of knee. In these patients with medial compartment disease (varus knee), medial open wedge osteotomy was performed using full thickness illiac crest grafts. These were called for follow–up after 2 years and clinico–radiological assesment was done. Although many surgeons of the west do not favour this osteotomy, we found the prosedure to be quite acceptable to our patients in whom the primary concern of the cost and squatting habits are well taken care of. The authors feel that this osteotomy is still relevant in the third world. Results do deteriorate with time but most patients consider the surgery satisfactory. Materials andMethods :The authors retrospectively reviewed 30 knees of patients who underwent medial open wedgeosteotomy for primary medial unicompartmental osteoarthritis of knee between 2012 and 2014. Cases of secondary osteoarthritis were excluded from the study. Weight beå long film X–rayswere obtained from all the symptomatic patientsand were assessed for TFA (tibia–femoral angle).Functional assessment was carried out at the end of 2 years post–operatively. Results :Results have shown that medial compartment open wedge high tibial osteotomy is very effective in the management of osteoarthritis of knee as far as pain relief, joint stability, quality of life and patient satisfaction is considered. Conclusion :The authors conclude that medial opening proximal tibial osteotomy is a physiologicallybetter surgery in medial compartment primary osteoarthritis of knee in early stages. The longtermresults deteriorate with time but are stillgratifying enough for the Indian patients as well asfor those of the third world, who cannot affordthe costlier surgeries and implants and also arereluctant to change their squatting habits and job profile
