Objective: To share my experience of medical abortion in upto seven weeks pregnancies with previous caesarean scar on the uterus with mifepristone - misoprostal.Method: Records of women with previous caesarean section, who underwent medical abortion with mifepristone 200mg orally followed by misoprostol 800 mg vaginally 48 hours later, upto seven weeks pregnancy were analysed for scar rupture, excessive bleeding needing blood transfusion or hypovolumic shock, and failure of method. one hundred such women were analysed during the period of Aug. 2007 to March 2012.Result: Two cases of scar rupture with excessive bleeding and hypovoumic shock needing blood transfusion occured. These 2 women had history of 2 caesarian sections. Incomplete abortion occurred in 4% cases requiring surgical evacuation. There were 31% cases with 2 previous caesarean sections. Conclusion : This analysis suggests that medical abortion in women with previous caesarian section needs special care and specific guidelines need to be put in place to make it safe.
