NUTRITIONAL ASSESSMENT IN CONTINUOUS AMBULATORY PERITONEAL DIALYSIS (C.A.P.D.) PATIENTS

Abstract :

BACKGROUND: Nutritional status is an important factor in long term continuous ambulatory peritoneal dialysis (CAPD) prognoses. Conventionally, serum albumin is an indicator of nutritional status. More recently, protein catabolic rate and lean body mass have been calculated during peritoneal equiliium testing to assess nutritional status. These have the advantage of providing an early indication of malnutrition before the serum albumin decreases. The aim of this study to assess the nutritional status of CAPD patients at Maharani Laxmi Bai Medical College, Jhansi (Uttar Pradesh) using anthropometry subjective global assessment and biochemical methods. Methods: This cross–sectional study was conducted at Department of Medicine, M.L.B. Medical College, Jhansi, It involved 50 adult continuous ambulatory peritoneal dialysis (CAPD) patients who was receiving treatment on an out–patient and in–patient basis at the Nephrology unit of Medicine department of the M.L.B. Medical College, and who satisfied the inclusion criteria and present to department for Continuous Ambulatory Peritoneal Dialysis. Results: In study group, majority of the cases was in the age group 31–40 years was 13 (26%) followed by 41–50 and 51–60 11 (22%) respectively. In age group 61–70 years, 8 (16%) cases was found and only 1 (2%) cases was found in >70 years age group. Majority of the patients was male i.e., 32 (64%) out of 50 and 18 (36%) patients was females. The mean calorie intake of the patients on CAPD according to body weight is 1512.73+204.82 and mean protein intake of the patient was 61.23+9.89. Despict that the mean weight of the patients on CAPD was 58.06+7.47. the mean BMI of the patients on CAPD was 23.20+2.59. the mean Mid–upper arm circumference (MUAC) of the patients on CAPD was 23.70+1.79. The mean cholesterol level of the patient on CAPD was 189.83+29.19. the mean triglyceride level of the patient on CAPD was 149.77+33.20. the mean High–density lipoprotein (HDL) level of the patient on CAPD was 53.14+5.96. the mean LDL level of the patient on CAPD was 106.74+28.17. The mean of Serum creatinine level of the patient on CAPD was 9.77+3.84. The mean of S. Albumin level of the patient was 3.66+0.56. Overall Subjective global assessment (SGA) scores in this study showed that 25 (50%) of the patients was well nourished, 23 (46%) was moderately undernourished and 2 (4%) was severely undernourished. Patients according to Body mass index (BMI) showed that majority of patients 24 (48%) was found normal followed by overweight 17 (34%). No any patients was found in this study. According to ISRNM criteria 25 (50%) patients was malnourished. Conclusion: We concluded that malnutrition is a major issue among CAPD patients in Indian settings. Almost upto 50% of our study group are undernourished. Thus health education for patients and their families is needed about the nutritional requirements in the patients so as to increase their nutritional status and fulfill the recommended energy and protein intake to improve their quality of life.
