PROGNOSTIC AND CLINICAL IMPLICATIONS OF RATIO OF LEFT VENTRICULAR PEAK E-WAVE VELOCITY TO FLOW PROPAGATION VELOCITY ASSESSED BY COLOR M-MODE DOPPLER ECHOCARDIOGRAPHY IN FIRST MYOCARDIAL INFARCTION

Abstract :

Acute Myocardial Infarction (AMI) is a major health problem globally. The role of systolic dysfunction in prognostication of AMI is already established but the short term prognostic implication of diastolic dysfunction in AMI is not well studied. The ratio E/Vp>1.5 is an established predictor of LVEDP but its clinical role in stratification after AMI has not been previously reported . So, we intended to study the same. A total of 108 AMI patients admitted in the department of cardiology, from November 2018 to November 2019 were enrolled in the study. Echocardiography was done within 24hrs of arrival. The patients were divided into two groups of E/Vp<1.5 and E/Vp ≥ 1.5 and analysed for outcome. In AMI cases the E/Vp ≥1.5 was found to be a strong predictor of in–hospital heart failure. The ratio of peak E–wave velocity to flow propagation velocity is also superior to systolic variables in predicting in hospital cardiac death. However for in hospital death DT<140msec scored over E/Vp. The results suggest that these Doppler variables may be simple tools to rapidly risk stratify patients with first AMI.
