Spatial Disparity and Barriers in Utilization of Antenatal Care among Ever Married Women in India

Abstract :

 Background: An estimated half million women, each year, die from complications related to childbirth either during pregnancy, delivery or subsequently within 42 days of delivery. 99 % of the total maternal deaths occur in developing world, and India alone accounts for 22 % of the total global maternal deaths and these deaths can be prevented with key health interventions, like provision of antenatal care. Methodology/Finding: This paper investigates regional differences in utilization of antenatal care as well as identifies the determinants of utilization of at least four antenatal care visits among ever married women in India. The data for present study has been drawn from three rounds of National Family Health Survey (NFHS) conducted during 1992–93, 1998–99 and 2005–06 and other countries available data (DHS). Bivariate, Chi–square test and multiple logistic regression analysis have been used to understand the trends, patterns and predictors of the outcome variables. Findings indicate that India (37 %) is far away from reaching its goal of improving utilization of antenatal care services at the world level (55.1 %). Wealth index and education are main significant predictors for under utilization of ANC services in India. Higher educated and richest women are more likely to receive antenatal care services in all regions. Religion, exposure to mass media, birth order, age of women and place of residence are other predictor of utilization of ANC services. Worse performing regions in India are northeast, central and eastern region where quality and quantity of receiving antenatal service are poor. Conclusion: The results of our analysis, which indentified that uneducated woman, women from poor family, higher parity, rural and younger women are less utilizing at least four ANC visits. Government’s policy should immediately focus on poor and uneducated rural women and also reduce the direct and indirect cost of visiting health facility
