SPONTANEOUS RUPTURE OF URINARY BLADDER -A RARE COMPLICATION OF ACUTE CYSTITIS

Abstract :

Spontaneous rupture of urinary bladder is a rare clinical entity that requires immediate medical attention due to its high morbidity and mortality. It presents a diagnostic challenge to the clinician. We report this case of a 78 year old lady diagnosed as a case of acute cystitis associated rupture of bladder. She presented to us with sudden onset abdominal pain and rapidly progressive distension with features of peritonitis. Abdominal imaging and biochemical analysis of ascitic fluid revealed a urinary ascites. Open repair of bladder was done. Biopsy from the bladder tissue indicated acute cystitis. Post–operative cystogram showed excellent healing of the bladder. SUMMARY; While bladder rupture may occur in association with blunt or penetrating lower abdominal and pelvic injuries, spontaneous rupture of bladder is rare. Spontaneous rupture of bladder has been known to occur in a cancer beå bladder, neurogenic bladders or in post irradiation bladder. There have been case reports on bladder rupture associated with Diabetes mellitus, acute alcoholic binge and tubercular cystitis.We present this rare case of spontaneous rupture of bladder that developed as a complication of acute pyogenic cystitis and presented to us as an acute abdomen with features mimicking acute renal failure
