Study of Intrastromal voriconazole in resistant fungal corneal ulcer.

Abstract :

AIM: To investigate the role of intrastromal voriconazole in resistant fungal keratitis. METHODOLOGY: This hospital based prospective interventional study (nonrandomized) was conducted in Malda Medical College, West Bengal, India from June 2018 to November 2019 where 15 eyes of 15 patients with resistant fungal keratitis were included. All the patients were previously treated with topical Natamycin and topical Voriconazole along with systemic Ketoconazole and were not satisfactorily improved. Institutional clearance and written informed consent from each patient were taken before giving intrastromal voriconazole injection. Complete resolution of corneal infiltrate with formation of scar defined as success while worsening of symptoms and necessity for therapeutic penetrating keratoplasty indicates failure. All the patients were observed for at least three months. RESULTS: Eleven patients had shown success as there was complete resolution of corneal infiltrate and formation of corneal scar. Four patients did not respond to intrastromal voriconazole and progress to corneal perforation ultimately needed therapeutic penetrating keratoplasty. Corneal ulcer size and height of hypopyon were significantly associated with these failed cases. CONCLUSION: Intrastromal voriconazole is a good therapeutic option in resistant fungal keratitis and reduce the necessity of Penetrating therapeutic keratoplasty.
