TO STUDY THE EFFECT OF ORAL EPHEDRINE ON HAEMODYNAMIC RESPONSE AND NEONATAL APGAR SCORE IN PATIENTS UNDERGOING ELECTIVE CAESEAREAN SECTION UNDER SPINAL ANAESTHESIA

Abstract :

Background: Spinal anaesthesia induced hypotension is a well known complication. Several attempts have been made to find the ideal intervention to ameliorate this effect with varying results. Studies describing oral route to prevent this complication in caesarean section are sparse. Methodology: Present study is a prospective, randomized, double blind, controlled trial where 100 patients belonging to ASA Grade II undergoing cesearean section under spinal anaesthesia were randomly allocated into two groups of 50 each. Group E received prophylactic Oral Ephedrine 30mg and Group P received a Placebo (control) with a sip of water 30minutes before SAB with 2ml of 0.5% hyperbaric bupivacaine hydrochloride. Results: Patients were comparable with demographic data and baseline hemodynamic parameters. Compared to group E, group P had greater incidence of hypotension and greater number of patients required vasopressor therapy. Non hemodynamic side effect were also greater in group P. Neonatal outcome was good in both group as compared by APGAR score. Conclusion: The prophylactic administration of oral Ephedrine in ASA Grade II patients undergoing caesarean section under spinal anaesthesia is an effective measure in preventing hypotension without causing untoward side effects like central nervous system stimulation, tachycardia or arrhythmias.
