Vimentin expression in Invasive Duct Carcinoma of breast and its clinicopathological correlation.

Abstract :

Introduction: Breast carcinoma is the most malignant tumor and the leading cause of carcinoma deaths in women. Its etiology is multifactorial, implicating reproductive factors, hormonal imbalances and age. Epithelial to mesenchymal transition ( EMT) is involved in cancer progression, particularly during invasion and intravasation when tumor cells migrate to distant organs to form metastases. Vimentin is expressed in mesenchymal origin of cells and also expressed in epithelial cells undergoing EMT under both physiological and pathological situations. Aim and objectives: 1).To assess Vimentin in Invasive Duct Carcinoma of Breast by immunohistochemistry. 2).Clinicopathological correlation of Vimentin positivity with prognostic factors (Nottingham Prognostic Index) in Invasive Duct Carcinoma of Breast. 3).Correlation of Vimentin expression with ER ,PR, and Her 2 neu status. Materials and Methods: Radical mastectomy specimens were studied for Vimentin expression by immunohistochemistry in 50 patients diagnosed as Invasive duct carcinoma in Department of Pathology, SMS medical college, Jaipur. Immunohistochemistry was done using antibodies against vimentin, ER, PR and Her–2–neu. Vimentin expression is quantified as Vimentin Score. Relationship between Vimentin expression with various clinicopathological parameters was evaluated. Statistically analyzed by Chi square test/ Fishers exact test and Mann Whitney U test. P value less than 0.05 will be taken as significant. Results: Vimentin expression was seen 27(45%) of cases. Its expression correlated with high histological grade, lymphovascular invasion, negative ER/PR status and triple negative status (P value <0.05). A significant correlation was found between NPI and vimentin expression (P value <0.05). It did not correlate with age, tumor size, lymph node status and Her–2–neu status. Conclusions:Vimentin expression is strongly associated with poor prognostic factors of invasive duct carcinoma which may be a potential target for cancer therapy.
