A RARE CASE OF COMBINATION OF CONGENITAL AND ACQUIRED HEART DISEASE : LUTEMBACHER‘S SYNDROME

Abstract :

Any combination of ASD (congenital or iatrogenic) and MS (congenital or acquired) is referred to as Lutembacher’s syndrome (LS)1. Incidence of LS is 0.001 per million populations, while the proportion of iatrogenic LS stands at 11–12%2. A 26–year multiparous woman presented with complaints of progressive pedal edema and dyspnea on exertion. Diagnosed as chronic rheumatic heart disease during first pregnancy. On examination , she had facial puffiness, bilateral pedal edema , raised JVP , a displaced apex beat in the 6th intercostal space, palpable first heart sound, palpable pulmonary component of second heart sound and parasternal heave .On auscultation, she had a loud first heart sound and a mid–diastolic murmur in the mitral area . We suspected the patient to have secondary pulmonary arterial hypertension due to mitral stenosis and right–sided heart failure along with acyanotic congenital heart disease like ASD and confirmed by 2d ECHO
