Devic’s disease in HIV patient: a case report.

Abstract :

 Neuromyelitis optica (NMO) is a heterogeneous disorder with simultaneous inflammation and demyelination of the optic nerve and the spinal cord. A 37 years old female came to emergency department with complaints of progressive weakness of left upper limb and lower limb of 5 days duration. She had a history of blurring of vision of right eye for the past 1 month. She is a known case of retroviral disease with HIV–1 positive for 10 years and on ART TLE regimen from 2 years. MRI T2 weighted hyper–intensity in spinal cord extending medulla to C2 stpinal segment levels with no enhancement post contrast suggestive of longitudinally extending transverse myelitis. NMO–IgG antibody or Aquaporin–4 antibody test was positive. She improved dramatically with steroids. She was again readmitted with a relapse with quadriparesis and blurring of vision of right eye. She responded to steroids and plasmaparesis.
