Evaluation of Visual Outcome among Traumatic Optic Neuropathy Patients Attending a Tertiary Care Hospital of Raipur City, Chhattisgarh

Abstract :

Introduction– The most common cause of traumatic optic neuropathy (TON) is indirect injury to the optic nerve. Incidence of Traumatic optic neuropathy (TON) reported in published case series was ranged from 0.7% to 2.5%. The diagnosis of TON is made easily on basis of clinical findings and history. Early diagnosis and prompt intervention could be improved the outcome as per previous studies. So the current study was planned to evaluate the outcome of traumatic optic neuropathy cases in tertiary care hospital of Raipur City. Material and Methods– The present prospective study was conducted in Dr. BRAM Hospital, Pt .J.N.M. Medical College, Raipur (C.G.) India, during study period April 2013 to April 2015. Current study included 20 patients with indirect optic injuries. All patients were categorized in to two groups – Group A included 12 patients who received treatment within 7 days of injury and group B included 8 patients who received treatment after 7 days of injury. All enrolled cases underwent complete ocular examination including visual acuity test, intraocular pressure measurement, decreased color vision, pupil assessed for relative afferent pupillary defect (grades), visual field deficit, ocular motility and fundus examination. Data was compiled in MS Excel and checked for its completeness and correctness. Then it was analyzed using online statistical calculator and chi square test were applied Results– This prospective study included 20 patients with unilateral traumatic optic neuropathy (TON). In both groups there was a male preponderance (80% in group A &100% in group B). Right eye was involved more in group A but left eye was targeted in group B. Group A which receives treatment within 7 days of injury achieved 100 % improvement in vision (p<0.0001) whereas there was improvement of only 50% patients in whom treatment started after 7 days of injury. Conclusion– Present study reveals that time lapse between injury and treatment could be a better prognostic factor for traumatic optic neuropathy cases.
