Radial Corrective Osteotomy and Sauve–Kapandji Procedure in The Management of Symptomatic Malunited Colles Fracture : A Clinical Study

Abstract :

Objective: To evaluate the results of corrective osteotomy and sauve–kapandji procedure in the management of symptomatic malunited colles fractures. Material and methods: Eight patients with symptomatic malunited colles fractures were included in our study. Radial corrective osteotomy along with Sauve–Kapandji procedure was done in all eight patients, after taking consent and under G.A. They were followed at two weeks, six weeks, twelve weeks, twenty four weeks and one year. Severity of pain, functional status, hand grip strength, and ROM both in DRUJ (distal radio ulnar joint) and RCJ (radio carpal joint) were recorded and compared to normal side using Cooney’s clinical scoring chart. Results: Out of eight patients six patients were male and two were female, the mean age was 38 years (range 20 – 54 years) The follow up was done from six months to twelve months. Seven patients showed excellent or good results and one patient showed fair results because we failed to achieve complete pseudarthrosis at resected ulnar site. But had to change his occupation due to discomfort in DRUJ. A gain in DF and PF range was 30%. All patients regained full pronation and supination except in one with fair results. The grip strength improved in all cases but it was slightly short of normal side. Cooney’s score evaluated that seven had scored more than 80 points and in one patient it was 65 points. Conclusion: We concluded that results were encouraging following corrective radial osteotomy and Sauve–Kapandji. procedure in the management of malunited colles fracture. Pain was relieved, hand grip strength and RCJ and DRUJ movements improved and cosmetically acceptable.
