Use of End Tidal CO2 as an alternative to Partial Pressure of CO2 in patients undergoing craniotomy.

Abstract :

OBJECTIVE: To determine the relationship betweenpartial pressure of CO2 (PaCO2)and end tidal carbon dioxide (ETCO2)during craniotomies in supine position.   METHODS: This prospective observational study included adult patient above 18 years of age.Samples ofABGfor PaCO2were taken at10 minutes after induction of general anesthesia, after craniotomy and prior to dural incision, beginning of dural closure and before extubation and ETCO2 was recorded simultaneously. Correlation and agreement betweenETCO2 and PaCO2were analysed and Bland Altman plot was constructed. P< 0.05 was considered significant.   RESULTS: Seventy patients were included with mean age of 44.2±11.9 years.Mean ETCO2 measuredwas lower than the corresponding PaCO2. A significant (p–0.000) positive correlation exists between the PaCO2 and ETCO2. Bland Altman plot showed good agreement between PaCO2 and ETCO2values.   CONCLUSION: We conclude that ETCO2can be usedas an alternative of PaCO2 for neurosurgical procedures in healthy patients.
